1. Full Name of Patients and Attendants
2. City
3. Arrival date & time
4. Duration of treatment
5. Type of Room

Once you send the booking details then we will send you details for the advance payment.

STEP 2: ADVANCE PAYMENT TO RESERVE YOUR APPOINTMENT

To confirm your booking please transfer above mentioned amount at given UPI ID 👆🏼 or at the given QR code 👇🏼. Once the payment is successful then please send us a detailed receipt with the transaction ID through WhatsApp. Thereafter we will confirm your booking.

Note: The advance amount what you pay will be deducted from your total bill. You can also get original receipt at your arrival. The advance amount is non-refundable, however you can reschedule your appointment in the next 6 months.
